I often hear OB's approve of patients having 5 oz of wine is okay during pregnancy. Is this true? and if so how often and at how many weeks is this okay?

Welcome, everyone. Thanks for your interest in having a safe and healthy pregnancy. Let's get to your questions.There is no quantity of alcohol that has been established to be safe in pregnancy. I tell my patients they should consume no alcohol.

There is very good information that tells us that proper dental hygiene along with regular dental appointments reduces the risk of pre-term delivery. Dental care during pregnancy is generally safe, however, this should be discussed with your OB and your dental provider.

I had a miscarriage last year and I'm worried that it could happen again. Am I at higher risk for having another miscarriage? Are there any warning signs that I can look out for?

Sadly, almost 25 percent of pregnancies will result in an early miscarriage. Having had one miscarriage does not necessarily put you at higher risk. However, if you have had multiple miscarriages I would recommend that  you seek the advice of an OB for further information.

Great question, Chelsea. The recommendation for non-high risk patients is 0.4 milligrams of folic acid. For vegetarians and women with other medical problems, additional supplements may be required. Talk with your OB about what would make sense for you.

I get my hair professionally colored, and I was wondering if it is safe to continue getting it dyed when I become pregnant? Since there's chemicals that could potentially touch my scalp involved, I want to make sure it's not dangerous or toxic during pregnancy. Thanks!

Jenna, there is limited data surrounding this topic, but it is a very common question. Therefore, I generally recommend that patients wait until after the first trimester to proceed with hair coloring.

Hi Dr. Jaspan!  I'm reading that Tylenol and Benadryl/Zyrtec are safe during pregnancy. I typically take Allegra D daily. Is Zyrtec safe for daily use or should it be strictly on an as needed basis when allergies get extremely bad?

Amanda, medications are graded by the FDA to determine their safety during pregnancy. Category A are generally acceptable and have been studied in pregnant women to show no evidence of fetal risk. Category B may be acceptable. Animals study show minimal risk, human studies are not available. We generally feel that these drugs are safe. Category C are used with caution and should only be taken if the benefit of the medication outweighs the risk. The risk will be identified in animal studies. Human studies are not available. Zyrtec is a Category B drug and therefore would be safe. Zyrtec-D is noted to be Category C and therefore should only be taken after discussion with your OB.

I was recently prescribed 200 mg of progesterone due to a luteal phase defect.   My progesterone was 8.9 at 7dpo and I started taking it right away.  I'm now pregnant, 6 weeks tomorrow, and my first 2 progesterone draws were 31 and 38.  Is this too high?  My doctor states I can reduce to 100 mg, but I'm nervous due to previous losses.  What are your thoughts?

Genevieve, it is safe to get manicures and pedicures while pregnant. One risk might be if you develop an infection in your cuticle that was not properly managed. As always, you should have your cosmetic procedures at a reputable location.

Guest, there is some controversy surrounding luteal phase defect. Therefore, each case requires individual attention. I would strongly recommend you communicate your concerns with your health provider.

I’m on methadone.  Is it safe to take during pregnancy, including in the first trimester?

Patients on methadone should not discontinue use unless under the supervision of a physician. There is a clinic run by Thomas Jefferson University that specializes in pregnant women on methadone.

How does having pelvic floor issues from 1st pregnancy that are not resolved impact a future pregnancy?

Marie, depending on the degree of the pelvic floor issue there may be a risk of pre-term delivery. The pregnancy may make your pelvic floor symptoms worse,  however, you should also know that if you have had surgical correction and subsequently become pregnant, surgical repair may fail.

JPM, generally speaking that is not the case. Fibroids, depending on their location in the uterus, may impact the baby's ability to be delivered head first.

Are there any exercises I can do to relieve the pelvic girdle pain I have? Sitting for long periods of time is uncomfortable and walking for more than 30 minutes can lay me out for days...

Suzanne, great question. I fully advocate for physical therapy to provide specific exercises for your condition, after clearing it with your OB. In addition, after speaking to your OB, massage therapy may also provide a benefit.

If someone takes birth control for a while and stops because they are now trying to conceive, is it normal for their menstrual cycle do be irregular/very late?  Is there any medication that is prescribed for this?

Lisa, it is common once a woman stops birth control for her to experience some irregularity to her cycle. No specific medication is required unless one fails to conceive in the next six to 12 months.

Diamond, in an otherwise healthy pregnancy and for women who were working out prior to pregnancy, we recommed that exercise continue throughout the pregnancy. However, after the second trimester, the center of gravity shifts and we ask women to use stationary bikes and avoid fall risk.

Is facewash with salicylic acid safe? I know creams are not recommended but what about a face wash that rinses right off?

Great question. I recognize that many over-the-counter products contain salicylic acid. However, by the book, this is a Category C drug and there are no human studies that address this product. We recommend using with caution.

I'm 40 pounds overweight and want to try and get pregnant. Will my weight put me at high risk for a premature delivery or other complications?

Claire, certainly pregnancy is a time when we want our patients to be as healthy as possible, so we would not recommend weight loss. However, for women who are overweight, we do not recommend as much weight gain as we would for women of appropriate weight for their height. There are certainly risks associated with obesity in pregnancy. Therefore, an open and honest discussion with your OB would be prudent.

My husband would like to me verify that it's not recommended for a woman to change the cat litter while pregnant.  (I know there is information out there but this has been his only pregnancy question and I feel obligated to ask.)
