How is injury recovery handled by teams? Is it kept entirely in-house, or do teams have outside doctors they trust? For longer-term injuries, are players coming into the stadium for daily rehab, or is it all entirely dependent on the injury?

Teams have their own medical/rehab staff, but also have a group of specialists who are affiliated with hospitals or private practices. For the most part, rehabs are done at a rehab place or the spring facility.

You claim in ebis, and then the results of the claim (success, someone ahead of you claimed and failed) is another email.

How is the player notified when claimed on waivers?  Does the claiming team call the player directly or his agent?

Yeah, anything like that, somebody from the team calls the player to welcome them, and then the travel person will get in touch with them to arrange getting them to the new town.
