Fans hate when a player doesn't hustle, but extra effort is how players get hurt. Kiermaier always seems to miss 60 games for a dive, Ramos will be on the DL for the non-waiver deadline, Tatis Jr. is out because of his slide. On the other hand, Gary Sánchez's performance last night must have been infuriating to watch. Have teams tried to figure out the appropriate level of effort (probably as a function of leverage), or is it too messy an area of individual psychology and not wanting to mess with instincts?

For the most part it's just going to be instinctual. You can't program a player to hustle when the leverage index reaches a certain point. You're absolutely right that it's always a balance and it's worth noting that Ichiro might not have ever attempted a diving catch for this reason. He values his health too highly

The odds of getting hurt while hustling are low. At the bare minimum, players should demonstrate "false" hustle -- running at like 90-95%
Injuries tend to happen when you're going all out. Players definitely shouldn't go all out under all circumstances. Sanchez yesterday? Should've. Unless he was still feeling hurt, in which case he should've told someone ahead of time

Because of his previous peak. Because he was sidelined this year with a non-arm injury. Even last season, Britton kept his ERA under 3 while his forearm hurt. Not very many relievers proven to be effective over multiple seasons

He's not on the 40-man roster, which is one problem, and another problem is that his strike rates are indeed a few points below the league averages. Legitimate reason to believe Sheffield isn't ready, so bumping someone off the 40-man roster to make room could be hasty
They'll re-evaluate if they don't trade for a starter soon, but for the time being I don't think Sheffield is the best option

Can Colorado keep up this pitching run they're on.  Last 30 days they're 4th best ERA both starters and relievers?  Big reason why they're 15-6 last 21 games with a brutal schedule.
