The injury concern is always in the back of your head a bit, and you'd rather all of your players get a blow, but it's really not a big deal.

They did. You figure out your needs, get the budget from the owner and start trying to find fits. Make priorities, and be ready to adjust on the fly.

Do team have robust models to predict injury risk? For instance can they take a players injury history, body type, etc. and spit out x% chance this player gets injured this season?

No. Teams have tried VERY hard to create them, but all they're really found is that players who stay healthy tend to stay healthy and players who get injured tend to get injured again, and TEND is carrying a lot of water.

How much extra does a prospect that was drafted late with very limited expectations need to accomplish to win playing time over a "blue chip" prospect that has under performed thus far?

Not a ton. In a good PD system, it should be absolutely merit based, with draft status having nothing to do with it.

Money has to be the obvious #1 predictor of where a FA will sign, but what's second: geographic location? Team is contending? Playing time? Or are those all so minor that it's really (mostly) all about money?

It's almost nearly 100% about the money. Geography can play a role, as can where the team does their spring.

The field itself? Not really. Certainly in the minors, but big league fields tend to be impeccable. I know some places with players don't like the lights or something, but not the field itself.
