The one thing that Smoltz has said that I think is interesting is that he thinks you can teach pitchers to pitch third time through the order. Even Smoltz had a TTO penalty, not that large, .632/.642/.680 OPS 1st/2nd/3rd. But does anyone think this is something you can teach, rather that just an issue of familiarity and pitching reperatoire?

I think you could work on pacing and deepening an arsenal, but ya familiarity is going to carry the day most of the time. Especially since teams don't tell pitchers to pace themselves. They want full bore for whatever they can give as opposed to pacing themselves to shoot for 6-7 IP regularly
