Teams book their own. It's incredible. I've travelled with a team about five times in my career and it's surreal.

When considering pitch counts, does the quality or type of pitches factor in? Or is it pretty much a set number?

Quality and type tend not to matter, but stress pitches do VERY much. 100 over eight innings is VERY DIFFERENT than 100 over five.
